COVID-19 vaccination of priority groups

We are planning for our COVID-19 vaccination service to be open to eligible patients from the middle of the week commencing 11th January 2021 onwards and we will soon start booking our patients into the dedicated clinic accordingly. However, there is a chance that the service will not be available until the following week, and if this is the case, we may need to rearrange appointments.

Please do not contact the practice to request a vaccine, we will contact patients in accordance with the national vaccination schedule.

We are not able to order vaccines, we can only provide vaccines as we receive them, so it will take time to vaccinate everyone. Please continue to follow the Hands, Face, Space guidance that is so very important.

Thank you for your patience and understanding.

Chiddenbrook Surgery welcomes new patients; provided you live inside the Practice area we are happy to register you with the practice. We are situated at the west of Crediton.

We cover Crediton and the outlying villages, to the north Morchard Bishop and Woolfardisworthy to the east Stockleigh Pomeroy, Newton St Cyres, almost as far as Cowley; in the south Tedburn St Mary and Hittisleigh and in the west Colebrooke, to the centre of Bow, Down St. Mary, Copplestone, and up to the Railway bridge an the A377 to the east of Lapford.

Registering with Chiddenbrook

To register, first contact the surgery by email or phone to tell us your postcode and road or street name. From this information we can confirm that you live within the practice area.

Next complete the New Patient Questionnaire and Registration Form found on the Pre-registration page.  Print this off and bring it to the surgery, along with a form of identification, ideally something with photo ID like your driving licence or passport. If you are registering a child/children under the age of six years, you will also be given an additional form to fill in regarding immunisations, and the receptionist will also take details to pass on to the health visitor.

Your information will then be added to our clinical system and a request will be made to your previous GP so that your patient notes can be forwarded on to us. If you are on medication, you will need to see a doctor before a prescription can be issued.

Please read our Confidentiality at the Surgery information leaflet which explains our strict duty of confidentiality to all patients.
