COVID-19 vaccination of priority groups  

We are planning for our COVID-19 vaccination service to be open to eligible patients from the middle of the week commencing 11th January 2021 onwards and we will soon start booking our patients into the dedicated clinic accordingly. However, there is a chance that the service will not be available until the following week, and if this is the case, we may need to rearrange appointments.

Please do not contact the practice to request a vaccine, we will contact patients in accordance with the national vaccination schedule.

We are not able to order vaccines, we can only provide vaccines as we receive them, so it will take time to vaccinate everyone. Please continue to follow the Hands, Face, Space guidance that is so very important.

Online Services

Registering for Online Services (SystmOnline)

For a quick and convenient way to order your repeat medication, book appointments, view your coded medical record and change your address or contact details, why not register for SystmOnline? To register, see below, or ask at reception when you next come into the surgery. You will need to fill out a form and bring two forms of identification with you (one photo ID). Registering is straight forward and is likely to save you time in the future.  There is also an App available for Smartphones.

SystmOnline is a service that Chiddenbrook Surgery is providing to help patients manage some of their NHS health care needs via the internet. The services available include:

  • Appointment booking and cancellation
  • Ordering of repeat medication
  • Updating contact information

Registration

SystmOnline is open to patients aged 16 or over. To register you will need to complete our registration form below and provide 2 forms of identification (including 1 photo ID) for us to verify your identity. When patients register they will be able to access services securely online using a username and password. There is also apps available to enable access services via your smart phone or tablet.

Viewing Your Medical Record Online

To access your records you will need to complete the application form below and have read the information leaflet “Online Services Records Access, Patient information leaflet: It’s your choice”. Please note that patients will have to wait for records access as this requires GP approval. When requesting records access patients are asked to consider very seriously the implications outlined in the Patient Information Leaflet particularly with respect to coercion and confidentiality.

Patients will be advised separately once their request is approved for records access. Anyone already signed up (prior to 1st April 2016) for SystmOnline for medication ordering or appointment booking will still need to complete the application process to be able to access their records.

Please note that whilst we will make every effort to provide access on a timely basis, our priority will always be to patient care. Delays in processing requests for access may be experienced when we are busy.
